NO-COST HOME CARE ASSESSMENT & EVALUATION
Our Home Care Consultant Coordinator will make an appointment to meet with you and your family. She will gather information on your home care issues, service needs, and the qualities of the caregiver you seek. In this assessment visit, she will not only gather information, but will learn as much as possible so we can deliver the best service to meet your needs. The Assessment includes:
Client Personal Profile including Leisure and Behavior
Medical Profile/Medication
Nutrition Planning/Preferences/Dietary Restrictions.

PLAN OF CARE
After our assessment visit, our Care Coordinator will create an individualized Care Plan to be implemented by our Caregivers that we placed with to match each client's needs. It will also include the service plan that we will need the client's or family's input to be included and agreed upon. We want everyone to tell us what exactly they need and their expectations. We will only make suggestions if deemed necessary and for the best interest of the clients. Our Caregivers will be briefed with the plan of care so they will familiarize themselves on what is to be carried out for each particular assignment.

MATCHING WITH YOUR CAREGIVER
We try to give every client a "Perfect Match". We strongly believe and it is very important to us to make our client happy with a caregiver we placed. Some things we take into consideration in knowing our client's background and also the caregivers. Here are a few to mention:

And every other common grounds that a client and caregiver may share.
"WE DO NOT RECYCLE OR KEEP CHANGING CAREGIVERS". Once we placed someone and you are happy and satisfied, you will retain that caregiver(s) unless they need a break or an emergency arises, one of our Office Staff will come and fill in. " WE ARE CAREGIVERS OURSELVES"!

CAREGIVER INTRODUCTION & ORIENTATION
After briefing and explaining the plan of care to our caregivers, our Home Care Consultant Coordinator will come and introduce the caregiver(s). We allow up to a week or so for the client and caregiver to get to know each other then we will find out if the client is happy and satisfied. We won't rest until you are comfortable and enjoy your caregiver. The introduction is very important and caregivers are also trained in building relationships with the client. We love to see a smooth and comfortable transition between clients and caregivers.

ON-GOING CAREGIVER SUPERVISION
We support our Caregivers all throughout their placement with our Clients. We provide education and Training to keep their certifications and other relevant documentations up to par. We offer personal, professional and spiritual support whenever it is needed. Building our caregivers lives is very important and only then we can be sure they will do a great job and our Agency will be well represented.

ON-GOING FAMILY SUPPORT
We continue to support our clients and their family even after they signed up for service. We avail ourselves 24/7 for anything they may need or even just to talk and share issues and worries. If family members live far or out of State, we will update you with any changes or we have concerns that needs to be addressed right way. Communication is vital and we will keep you informed always.

LONG TERM CARE INSURANCE MANAGEMENT​
We work with any Long Term Care Insurance Companies. We are Licensed in The State of California and we will work with you to submit documentation needed so you can receive your payment in a timely manner.
